This form needs to be completed and on file with the Boat Ramp Administrator to obtain access to the lake for non-resident watercraft. Lake Sherwood “Member” identified below must be an active member in good standing. Members are responsible for ensuring their guests know our lake rules. A guest boat cannot be a watercraft that is owned by another LSA resident who (for any reason) is not an active member in good standing.
